Dear CMO leaders,

You were recently invited to participate in an online survey as part of the CMO Connect project being undertaken by the University of Newcastle with the support of the Mental Health Commission of NSW and the Mental Health Coordinating Council. For those organisations that have already completed the survey, we would like to thank you for contributing to this important research.

The University has asked us to let you know that as a token of their appreciation for your completion of the survey, your organisation has automatically been entered into a draw to win either a 2 day training course of your choice offered by the Mental Health Coordinating Council to the value of $500 or a voucher of $500 equivalent value which could be put towards equipment of your choice for your organisation.

This prize is being provided by the University of Newcastle. The recipient of the prize will be randomly selected at the completion of the survey period, and will be informed individually by research staff. For those of you who have not yet completed the survey there is still time! Completing the survey will still give you the opportunity to go in the draw for the above prize.

To complete the survey, please click on the link, or copy and paste the URL into your internet browser:  https://uonpsychology.au1.qualtrics.com/jfe/form/SV_896S9dcpCrgI11P

Additional survey instructions:

If someone else within the senior management of your organisation is better suited to complete this survey, please forward this email to them. When they click on the link they can identify their position within the organisation and their own email address when completing the survey.

A PDF version of the survey is also attached should you prefer to complete the survey by hardcopy and then scan and email it back to the CMO Connect team at cmoconnect@newcastle.edu.au

If you would prefer to complete the survey over-the-phone please contact the CMO Connect team on 4913 8123 or email cmoconnect@newcastle.edu.au to schedule a suitable time.

If you would like further information, please contact Prof Jenny Bowman on 02 4921 5958 or jenny.bowman@newcastle.edu.au

Once again, thank you very much for your participation.

Kind regards

Tania
